In 2026, the total altruistic surrogacy cost in India generally sits between ₹15 lakh and ₹35 lakh. Where you land inside that band depends on your medical needs and on the clinic that you choose. Roughly, it works out in three tiers.
The basic package runs about ₹15-20 lakh. That is a single IVF cycle using the intended parents' own eggs and sperm, with standard medical care, legal documentation and the surrogate expenses all included in it.
Need donor eggs or sperm? Extra monitoring, or more complex care? Then the case moves into the advanced package, and that is roughly ₹20-28 lakh.
₹28-35 lakh is the higher range. Multiple IVF cycles sit here. So does advanced fertility treatment, and the management of any pregnancy complications.

Why is the cost not a payment to the surrogate?
The Surrogacy (Regulation) Act, 2021 bans commercial surrogacy. A surrogate mother cannot be paid any fee, any salary, or any compensation.
Intended parents only cover a set of legitimate expenses that are documented. These include:
The surrogate's medical and pregnancy-related care.
Mandatory insurance which covers her for 36 months.
Her genuine pregnancy-related costs. Nutrition and medication, for example.
Any direct payment that goes beyond these specified expenses is a criminal offence under the Act. The phrase "surrogate mother cost in India" only refers to the reimbursed expenses and the insurance. It never means a purchase. It never means a payment for carrying the child.

The distinction is an ethical one. It is also the law. Paying a surrogate beyond her approved expenses carries serious criminal penalties under the Surrogacy (Regulation) Act, 2021.

Number of IVF cycles needed: This is the biggest single variable. A first-cycle success costs far less than several attempts.
Donor eggs or sperm add to it wherever they are medically indicated.
